Community Health Center Network (CHCN) was formed by Alameda County’s community health centers in 1996 in order to more effectively participate in newly launched state managed care programs.  As a management services organization, CHCN now provides a wide range of administrative, clinical, financial, and information technology management services to our member health centers.  CHCN includes eight community health centers all committed to providing comprehensive quality health care in a manner respectful of community traditions and values.  Our health centers operate 30 primary care sites in Alameda, Contra Costa, Solano, and San Francisco counties.  CHCN continues to evolve with the changing health care environment to better support our health centers as they strive to improve the health of the communities they serve.
